TO BE PRODUCED WHEN APPLYING FOR A VISA FOR
FAMILY
REUNION WITH PARENTS
(child
under 18 years of age / 21 years of age)
1) A valid passport:
To be obtained from the local Public Security Bureau. Please submit two
photocopies of your complete passport (all the filled pages).
2) Four application
forms signed and dated by the applicant. These forms are to be completed in
Dutch, French or English, preferably by typewriter. On each form a 2-inch
photograph should be affixed.
3) Two photocopies of
the applicant’s identity card (if applicable).
4) A notarized birth
certificate mentioning the names of both parents. *
5) In case one of
the parents has deceased: A notarized death certificate.*
Please submit two photocopies of the
documents under items 4 & 5. The original
certificates will be returned to you when the visa is granted.
6) In case the
parents are divorced:
a. a true certified
copy of the divorce decision mentioning to which of both parents’ charge the
child has been entrusted. *
b. a parental
authorization (in the notarized form) stating that the child can leave China
definitely.*
7) In case the child
has been adopted: A notarized adoption certificate.*
8) A certificate of
registration issued to the parents by the
municipality in Belgium and a photocopy of the parents’ Belgian identity card
or their residence permit in Belgium.
9) Depending on
circumstances, other documents may be demanded (such as DNA-test result),
particularly in case the relation between the parents and the child cannot
sufficiently be established.
10) Applicants may be
subjected to an interview if the Consulate General deems this necessary.
11) Two envelopes
with family name, first name, post code and address in Chinese and your
telephone number.
Note: All
documents (except those under 1, 2, 3) have to be produced in one original and one photocopy.
You should
submit all the required documents listed above in person Monday through
Thursday from 9:30 till 12:00 AM.
Visas can be picked up Monday through Friday from 14:00 till
16:30 PM.
Only complete files will be accepted.
Your dossier will be forwarded to the relevant authorities for
approval.
You will be informed in due time of the decision taken by these
authorities.
* This document has to be certified and translated by a notary public
and authenticated by the Shanghai Municipality Foreign Affairs Bureau (or the
relevant provincial Foreign Affairs Bureau) and this Consulate General.
